Overview

Winn-Wright provides person-centered case management to help eligible participants identify priorities, organize next steps, and connect with appropriate services after incarceration or housing instability.

The case manager works with the participant, while the participant remains involved in decisions and required follow-through.

  • Winn-Wright housing residents.
  • People facing several connected barriers to stability.
  • People who meet current program, funding, or referral requirements.

What is included

  • An initial needs and strengths assessment.
  • A person-centered service plan.
  • Goal setting and progress reviews.
  • Coordination related to housing, benefits, healthcare, documents, and employment.
  • Help organizing appointments and applications.
  • Referrals to qualified outside providers.
  • Follow-up on agreed next steps.

How the process works

  1. Submit a request or receive an approved referral.
  2. Staff review eligibility and current capacity.
  3. The participant completes an intake assessment.
  4. The participant and case manager identify priorities and create a plan.
  5. They meet according to an agreed schedule.
  6. The plan is updated as goals, needs, and available resources change.

Important limitations

  • Case management does not guarantee housing, benefits, employment, or acceptance by an outside provider.
  • Case managers do not provide medical treatment, psychotherapy, or legal advice.
  • Participants may need to complete applications and attend outside appointments.
  • Service frequency and duration depend on program requirements and capacity.
  • Urgent or crisis needs require an appropriate emergency or crisis provider.
